Fitting guide:
The orthosis should be fit with a thick sock as an interfacing. The upper lateral aspect of the orthosis should extend over the apex of the Fibula head to insure that there is no pressure on the perineal nerve. Otherwise the orthosis should be modified appropriately to relieve this area. You can see the carbon composite structure inside of the orthosis. The flexible shell of the orthosis will become malleable with use but the core structure will not and can not be heat molded in any way. Padding can be used to relive any impingement noted. Remember, if the patient is not comfortable in the fit of the orthosis the functional benefit will be negligible. Upon initial evaluation of the fit, if impingement is noted, going to a larger size may be needed. This will give room for padding adjustments and can accommodate growth better.
With the initial donning of the orthosis, have the patient stand in place. Use assistive devices for stability as needed. Instruct the patient to maintain their heels on the floor and to bend their knees slightly, about 5 degrees. Have the patient do this a few times before attempting to walk. This helps the patient to realize that they can bend the orthosis and for the patient to feel the increasing nature of the resistance force. Before walking, ask/insure that the patient is comfortable and is not experiencing any impingement from the orthosis during weight bearing.
